Dr. Wilderman: Some people come into the office complaining that they clench and grind their teeth at night. What they don't understand is why they're doing it. They think that it's stress-related. And what I say to them is that stress can precipitate the episode of you clenching and grinding your teeth, but it's not actually the cause. In general, the cause is due to a poor bite. And a poor bite can be the most destructive force in the mouth. I've seen people grind their teeth where they almost have no tooth left above the gum line.
And in cases of mild wear and tear of the teeth, we can create what's called a night guard. A night guard's a custom-fitted appliance. We take impressions of your top and bottom teeth and send them out to the laboratory and then they fabricate a plastic appliance. And when that comes back, we need to adjust it in your mouth to make sure that it's a comfortable fit, but most importantly, to adjust the bite so that now you have a perfect bite, except that it's simulated in plastic. And that will, in most cases, completely stop the need to clench and grind your teeth.
